How to book & What to bring

  • Some employers require a TB test, which involves two appointments scheduled 48 to 72 hours apart.


    When booking, please ensure you select both your initial test and the follow-up. If you don’t see availability for the follow-up online, contact us directly—we’ll do our best to accommodate you. Please note: you must be flexible within that 48–72 hour window and our business hours.

    • All immunization records (including from other countries). We have access to the online Alberta registry; however, sometimes those records are incomplete, contact your local health centre to get a copy if you do not have one. (780-413-7985 in Alberta or 811)

    • List of currently prescribed medications

    • Employment forms.

    • Completed Intake Form

  • Please arrive 15 minutes prior to your appointment *

    One of our nurses will go through your medical and vaccine history, ask about any medications being taken, chronic illness, check your requirements according to your program and start filling out your required immunization forms if applicable.

    The Nurse will perform a TB skin test (Tuberculosis / Mantoux Skin Test) if applicable, and vaccines accordingly. TB skin test is a Two-part test, you must be available to come back to our office for a follow up 48-72 hours after the test. Make sure a follow-up is booked for your return.

  • After your appointment, our friendly front desk team will process your payment and provide a receipt that includes a DIN (Drug Identification Number)—this is required for reimbursement through your Private Benefit Plan or Health Spending Account.

    We accept Visa, MasterCard, Amex, Interac (Debit), and e-transfer to admin@atlasimmunization.com.

    **Tip: Please keep your receipt safe—replacements are subject to a $15.00 reissue fee.

    Before your visit, make sure to fill out the top portion of your school form with your personal information. If you don’t have access to a printer, don’t worry—we have blank copies available at the clinic.

EMPLOYEES & JOB SEEKERS

  • Bring your government-issued ID or healthcare card, any workplace immunization or TB testing forms, and any immunization records you have. If you don’t have access to a printer, we have blank copies available in the clinic.

  • TB testing requires two appointments:

    • Visit 1: TB injection

    • Visit 2: Return within 48–72 hours for the reading
      If you're unable to find a follow-up appointment online, contact us directly—we’ll do our best to fit you in.

  • Yes. We provide receipts with Drug Identification Numbers (DINs), which are required by most private insurance plans and Health Spending Accounts. Lost receipts can be reissued for $15.

  • Often, yes. We can review your records, administer any required vaccines or tests, and complete your forms during your first appointment. Some services, like TB testing or vaccines requiring multiple doses, may require a follow-up.

  • Yes. We offer direct billing and bulk invoicing options for companies covering staff immunizations and TB testing. Contact us to set up a custom arrangement.

  • We typically have same-day or next-day appointments available. We prioritize employer referrals and group bookings for timely service.

  • Yes. We complete and return all required immunization or medical clearance forms, and can provide printed or digital copies for your HR files.
